1) Your Name
2) Have you received the Travel Declaration Form
sent by the school?
3) Did you travel out of Singapore during the
June holidays?
If yes,
4) Which country did you visit?
5) When did you travel?
6) When did you arrive back in Singapore?
7) Are you well today?
If you are not well,
8)(For student who is not well) When did you
start feeling unwell? How do you feel? Do you have fever,
sore throat, cough, runny nose, headache, etc?

As you guys know, the total number of confirmed H1N1 cases in Singapore now stands at 365.
